Public Information Request

  1. Information Needed
    List the information requested on the lines below. Be specific and include as much information as possible or clarification will be required. If you are requesting a motor vehicle accident report, you should identify your relationship to a person, vehicle and or property involved in the accident. You may be asked to provide proof of your relationship to the person, vehicle or property. State law requires that some of the information contained in the motor vehicle accident report is not releasable unless you have a qualifying relationship to a person, vehicle, property or the report in general.
  2. Request
    I hereby request the following information from the Custodian of Public Records of the Town of Northlake, Texas. I Understand that the information will be provided under the Open Records Act and that a fee may apply.
  3. Notification
    The Town is under no obligation to create a document to satisfy any request or to comply with a standing request for information. The information will be released only in accordance with the Public Information Act, which may require a determination as to confidentiality by the Texas Attorney General prior to a release. The Town has 10 business days in which to request such a determination.

    You will be notified per your requested method (chosen below), when or if your information is ready. If a fee is charged we will let you know at the time of notification. You may mail your payment to or drop off your payment at (drop box available for after hours) at:
